2 $30 per provider per month.
3 For Paper Claims/Mailed Statements the following charges apply; $1.00/Paper Claim, $.90 Per Statement, and $1.50 per Workers Comp Claim.
Electronic Patient Statements will be charged at $.59 per Patient Statement.
4 Migration is dependent upon the format of the data to be transferred. Based on the format, the migration may be free of charge or may require an additional fee.
